  • Created ‘Services Received at Former School’ section: 
  • Added special education, title I, speech/language, social work, other services, describe other services 
  • Created ‘Health’ section: 
  • Added preferred hospital, name and schedule for medication, emergency medical alerts (allergies or problems), alert expires date, physical limitations 
  • Created ‘Health Conditions’ section: 
  • Added severe allergies, peanut allergy, Anaphylactic Shock, heart conditions, seizure, Asthma, Diabetes, bee sting, cystic fibrosis, vision problems, hearing problems 
  • Created ‘Forms Submitted’ section: 
  • Added birth certificate, proof of residency, immunization, hearing and vision, concussion awareness, OPT out-exclude FOIA, OPT out media release, OPT out FERPA/Health Department, Handbook, Virtual Consent, Device contracts
